How to Send Us Your Prescription

If your product order requires a prescription, we have several easy ways you may send it to us. Your prescription must be received before your order can be shipped to you.

 

By Email

If you already have a copy of your prescription, you may scan it or snap a picture with your phone and send it as an attachment to info@activelifestylestore.com. Please include your full name and order number in the email so we can quickly match it to your order.
 
 

By Fax Machine

You may fax your prescription to (877) 744-8306. Please include your name or order number to help us process it quickly.

 

Have us Contact your Doctor

If you would like us to request a copy of the prescription on your behalf, simply email the doctor’s full name, phone number, and your date of birth to info@activelifestylestore.com and we’ll be happy to complete that task for you.

If your CPAP or BiPAP machines has a heated tube/hose, then you might need an Adapter to connect it with your Zoey Automated CPAP Cleaner.  The adapter simply replaces the cover of your current humidifier chamber, allowing you to connect your heated tube.  A simple process and you only have to do it once!

 

NOTE:  If you do not see your PAP model listed below, or you do not use a Heated Hose, then you do not need an adapter to connect to Zoey.
